Payment for Services

Funding for clinic operations is provided by medical insurance, United Way of Erie County and donations.

Our team consist of unpaid volunteers. Therefore, all funds we collect are applied to continuing clinic operations.

On your first clinic visit, please bring your child's medical insurance cards and Pennsylvania Access card (if applicable), as well as your child's birth certificate (for identification purposes).

HMO/Insurance

If your child's medical insurance is a Health Maintenance Organization (HMO), you must contact your primary care physician and request that a Cleft Palate Clinic referral be submitted to our office before the scheduled clinic visit.

If you have other medical insurance, we will submit your clinic services statement to your insurance carrier. Monies paid directly to you by your insurance company for billed clinic services for your child must be sent to the Cleft Palate Clinic office. We will notify your insurance carrier if such monies are not forwarded to us.
